As part of the rollout of Microsoft 365 at Michigan Medicine, Microsoft Forms is now available to everyone in Michigan Medicine. This simple online survey, poll, and quiz builder can aid in collecting feedback, measuring satisfaction, and organizing events.
When you create a form, you can invite others to respond to it using a web browser or mobile device. As results are submitted, use the built-in analytics to evaluate responses or export the data to Excel for additional analysis.
How do I access Microsoft Forms? Go to office.com and log in with your Michigan Medicine (Level-2) credentials. There is no desktop application.

How does Microsoft Forms compare to Qualtrics?
- Qualtrics is a research software program that allows you to create surveys using sophisticated analytics. It should be used when collecting and filtering substantial amounts of data or when you require a variety of visual reporting options for more complex analytics. You can access it at umichumhs.qualtrics.com and log in with your UMICH (Level 1) credentials.
- Microsoft Forms is a Microsoft 365 application that allows you to easily create custom quizzes, surveys, questionnaires, and registrations. It integrates with other M365 tools, including SharePoint, Outlook, and Office 365. You should use it when you need a simple survey or quiz, as a registration tool for small events, and when surveying a smaller audience.
